STATE MANDATED POLICY

1. Pay lunch by the 5th of each month ($35.00)

2. Must pay daily ($1.75) if not paid by the month.

3. Milk is .35/day or may be paid monthly. If the money is not paid by the 5th or daily, the student will not be allowed to eat. If this is a problem, contact PEGGY HANSON, lunch program director, and make arrangements. THANK YOU FOR YOUR COOPERATION.

4. State policy requires a written note from the doctor to be put on file in the lunch room to excuse students with milk allergies from drinking milk with their lunch. Please send note to the school: ATTENTION PEGGY HANSON.

5. All meals are served with milk and fruit and are prepared at St. Jude Thaddeus School.
